Jen Bouchard

Owner & CEO / ANCC board-certified Family Nurse Practitioner. Institute of Functional Medicine Certified Provider

Jen loves the water, having spent most of her summers since childhood on a beautiful, family-owned lake property in Indiana and now lives in a beautiful community in Chagrin Falls, Ohio, with a lake in her backyard.

Her education spans decades—she earned her Bachelor of Science in Nursing degree through Valparaiso University in 1986; then, after working in the emergency room and labor and delivery spanning a period of 10 years, she went onward and upward to earn a Master of Science in Nursing and a degree as a Family Nurse Practitioner from Indiana Wesleyan University in 1996.  She received a Masters in Public Health, with a strong focus in environmental health and toxicology, after her work associated with toxin exposures in Louisiana from Tulane University in 2005.

Jen is an ANCC board-certified Family Nurse Practitioner with diverse advanced practice experience for more than 26 years. She is also a member of the American Association of Nurse Practitioners and one of nearly 1800 Institute of Functional Medicine (IFM) Certified Providers (IFMCP).

Through her work at the renowned Center for Functional Medicine at Cleveland Clinic, she has applied the "root cause" analysis to "functional oncology."  After watching her daughter experience the effects of cancer treatment and cope with a terminal diagnosis of cancer, she is striving to apply her functional medicine knowledge and experience to make a difference for patients with a cancer history.

Jen has a strong interest in and has practiced many forms of energy work daily for the last 25 years through of meditation, the Energy Codes, and Medical Qi Gong. 

She loves to cook and experiment with new flavors and foods and spends available free time socializing, including cooking delicious healthy meals in her home or wherever she might be with friends &/or significant other.  She offers these delicious meals to her family and friends and emphasizes the importance of healthy plant based organics and grass-fed meats. Jen also enjoys as much time as she can with her daughter, Caitlin, who is a level 1 Trauma nurse in Chicago and her dad who is approaching the end of life with dementia at the family lake home in Indiana.

Yvonne Mitchell

Nationally Board-Certified Health and Wellness Coach

Yvonne Is a Nationally Board-Certified Health and Wellness Coach. She is certified through the Institute of Functional Medicine. She is also a certified life and health coach, grief recovery specialist, Hypnotist and HeartMath and Mindfulness Intervention practitioner. She began her career in coaching by developing mentorship coaching programs for upwardly mobile employees in a large government agency (SSA) while serving as its national director of employee development and training. She later started her own successful coaching business where, over the years, she helped clients address areas such as weight management, retirement planning, grief recovery, spiritual enhancement, habit control, relationships and more. Her passion for patient  advocacy especially in the areas of cancer support and prevention and other chronic conditions led her to focus on Health Coaching and then to further specialization in Functional Medicine.

Her successful careers and insatiable appetite for health education and practice led to a new career chapter as a health coach and workshop facilitator with the Cleveland Clinic Functional Medicine Division. During those five years, she worked with thousands of patients to help them address all manner of health-related conditions. She served as ally, facilitator and coach for their health restoration journeys. She now brings all these experiences and the wisdom gained to Hyperion Functional Medicine to deliver a full spectrum of health and wellness coaching for her patients. She helps them get to the core of their health challenges, successfully implement modifiable lifestyle factors such as nutrition, stress, restorative sleep, exercise, movement and provider recommendations to optimize their health and longevity.

When not pursuing additional education and certifications, she serves as a volunteer chaplain in her church and as a patient advocate for family and friends. She has one son and two absolutely amazing grandchildren and loves dance, healthy eating and music and spends lots of time thoroughly enjoying these activities.

Tamara Moreal

Nutritionist

Tamara Moreal holds a Bachelor of Education and is a Registered Holistic Nutritionist. In addition, she continues to update her education in the areas of gluten, its function and implications on health, anti-inflammatory nutrition, hormone support and therapeutic elimination diets.

Tamara's desire to become a Holistic Nutritionist resulted from her health issues. Her symptoms of brain fog, fatigue, painful joints and gut dysbiosis at times were quite debilitating. These symptoms were resolved by finding and treating the root cause. She believes the client holds the key to their wellness and that true health involves a combined approach using natural nutrition, targeted supplements and lifestyle practices. Tamara's goal is to use a compassionate, manageable approach to empower her clients to take control of their health so they feel their best physically, mentally and spiritually. Tamara has consulted with many clients over the past several years to improve digestion and energy and decrease inflammation. She is genuinely concerned and cares about others. Tamara is personally motivated, reliable, accountable, confident in her abilities and well-prepared to assist others to meet their health and wellness goals! From her childhood upbringing on a working farm, she learned the value of knowing the source of food and learned to appreciate the land. In addition, took pride in cooking everything from scratch and eating seasonal produce. To this day, Tamara continues to grow vegetables, support local farmer's markets and enjoys pasture-raised meats.

Tamara is married to her husband, Michael, and has three children. She strives to maintain a balanced lifestyle with her active children and in her time enjoys, cooking meals, going for long walks with her dog and biking.
